By completing this form, you agree to:

(a) the collection, use and disclosure of personal information for the purposes of credit adjudication by our Leasing Partner and its funders; (b) and to enable the Leasing Partner and its assignees to provide leasing services; and (c) the Leasing Partner and its funders obtaining information from credit reporting agencies and listed references in conjuction with this application.
